Overview

We are recruiting a Band 7 Diabetes Clinical Specialist Nurse to work within Sefton Place Community Diabetes Team. The post holder will demonstrate a comprehensive knowledge base in relation to diabetes and promote excellence in nursing practice in a dynamic healthcare environment. The role includes leadership duties within the team, delivery of specialist nursing care, case management, and direct patient care as an expert practitioner to optimise patient health, provide education for self-care and manage complex patient pathways. The post holder will work within Sefton Place the Diabetes Team and provide a service to patients in Southport, Formby and the South Sefton area. Flexible working requests will be considered for all roles.

The post holder will be part of a multidisciplinary team at Mersey Care, which provides a wide range of services across the North West and is committed to delivering safe, effective, patient‑centred care. The organisation aims for high‑quality, integrated services and ongoing service transformation.

Responsibilities
  • Provide highly specialised clinical advice and support to service users and the wider healthcare team, including clinical assessment, diagnostic skills, non-medical prescribing, and implementation of management and treatment plans.
  • Be responsible for the delivery of the service within the area and work cooperatively with other services/stakeholders to ensure patients are treated in the most appropriate setting.
  • Ensure the team delivers a high quality service with caseload and clinical supervision; lead a team in the absence of the Team Leader.
  • Deliver specialist nursing care, case management, and direct patient care as an expert practitioner; optimise patient health and provide education for self-care and complex management of patient pathways.
  • Lead and mentor staff developing into a caseload management role; act as an advocate and champion for patients in professional groups and forums.
  • Develop and implement plans for the team, including rotas and schedules to ensure business continuity; practice autonomously with evidence‑based clinical decision making.
  • Provide clinical expertise to the wider healthcare team when managing complex and highly complex situations; assess conditions and consider options for complex care delivery.
  • Develop specialist, individualised care plans after holistic assessment with patient and carers involvement; implement and evaluate care delivery.
  • Ensure clinical activity reflects health promotion, supported self-care, disease management, long-term condition management, and palliative care where appropriate.
  • Set performance objectives and monitor progress; contribute to local service delivery through meetings and reporting outcomes to staff.
  • Support policy and service development, author protocols as required; provide induction to new staff and students; be an authorised signatory for timecards and mileage claims where applicable.
  • Monitor budgets, support PACE reviews, and contribute to policy development to reflect local needs.
  • Delegate administrative duties appropriately; provide data to monitor team contracts and objectives; participate in audits, research, supervision, and mentor/clinical supervision initiatives.
  • Maintain professional registration; support mentoring of students and identify skill deficits within the team with appropriate development plans.
  • Ensure effective record keeping and risk management; identify, report, and investigate incidents as delegated; monitor and ensure quality of patient care through audits and supervision.
  • Participate in patient satisfaction reporting to improve care and ensure services meet local needs.
