A healthcare provider in Chepstow is seeking a dedicated Registered Nurse to manage a caseload of patients and provide care coordination as part of a Multi-Disciplinary Team. The ideal candidate will ensure high-quality, user-centered care and actively contribute to the development of the adult mental health service in Lower Monmouthshire. This permanent position requires adherence to mental health measures and offers comprehensive training and professional development opportunities.
The successful applicant will work as part of the Multi-Disciplinary Team and Care Co-ordinate their own caseload of patients in line with the Mental Health (Wales) measure. You will be committed to ensuring the delivery of the highest quality user-centred care and contribute to the development of adult mental health service in Lower Monmouthshire.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The applicant will undertake core CMHT duties such as routine and urgent assessment clinics, assessing, planning and implementation of care. depot clinic, clozapine clinic and duty desk.
