Job summary

A community care service is seeking a Healthcare Assistant / Support Worker to provide person-centred care for individuals with complex needs. Responsibilities include assisting with personal care, clinical documentation, and nutritional support under the supervision of a Registered Nurse. Ideal candidates have relevant experience and strong interpersonal skills. This role offers flexible working hours and requires travel within the community. An Enhanced DBS check is necessary for application.

Qualifications

  • Previous experience in a Healthcare Assistant, Support Worker, or Care Assistant role.
  • Experience with personal care and daily living support.
  • Proven experience of manual handling and basic clinical procedures.
  • Knowledge of Positive Behavioural Support Principles and trauma-informed care.
  • Ability to work independently and escalate concerns appropriately.
  • Willingness to travel within the community.

Responsibilities

  • Providing safe, effective care under the supervision of a Registered Nurse.
  • Accurately recording care delivered and maintaining patient records.
  • Assisting with hygiene, grooming, dressing, and oral care.
  • Supporting feeding, hydration, and completing food and fluid charts.
  • Recording vital signs and escalating concerns promptly.
  • Maintaining a clean, safe environment and managing personal belongings.
  • Supporting comfort, rest, sleep, and emotional needs.
  • Emotional and behavioural support for individuals experiencing anxiety and distress.
