A leading mental health service provider in Southall seeks a motivated individual for the Discharge Hub at Ealing Hospital. The role involves overseeing patient flow and coordinating discharges to ensure safety and efficiency. The Ealing Hospital Discharge Hub has an exciting opportunity for an enthusiastic and motivated individual with expert knowledge of D2A/discharge pathways, discharge planning and experience working within a multi-disciplinary team. Your main role will be to have oversight of day to day patient flow through the discharge hub, coordinate and facilitate P1 to P3 discharges from Ealing Hospital into the community in a safe and timely manner. The role will be based at Ealing Hospital.
The successful post holder will work in partnership with colleagues in the acute sector, social services, Ealing Community Partners, ICB and other community health services to ensure that appropriate patients from the acute setting are assessed and transferred safely to appropriate community-based services in a timely and seamless manner.
