  • Hybrid working (3 days at home)

Preparing for Adulthood (PfA) Team sits in Bromley\'s 0-25 Service, which has been established to support the social care needs of children and young people with disabilities. We recognise that we need to improve the journey for young people with disabilities as they prepare for adulthood. We aim to support greater independence, promote education and employment, increase meaningful community inclusion and friendships, whilst supporting positive health and well-being. We seek a practitioner who is passionate about this, who is keen to be innovative and improve outcomes for young people.

The PfA Social Workers will work with young people from 14-25. From 14-18 this will be alongside the childcare Social Workers, thereafter, taking full case responsibility from 18 years of age. The PfA workers will hold case responsibility whilst an EHC plan is in place and until there is transition into adult care services. We are therefore seeking someone with experience in completing assessments under Care Act legislation and good skills and knowledge around assessing capacity and developing young person-centred care packages, whilst supporting families during this transitional period for the young person and family. You will be part of the wider 0-25 service and therefore an understanding of children services will be of benefit.

Requirements of the role
  • Qualified Social Worker
  • Fully Social Work England registered
  • Must have significant experience in adult social care
  • Must have experience of working with adults with disabilities or ideally in a Preparing for Adulthood / Transitions setting
