Position Summary: Under the general direction of the Site Manager, Supervisor, or Community Coordinator, the Direct Support Professional works with, trains, and provides active treatment and assistance to individuals according to their Individual Plan (IP), within the policies of The Society.

Essential Functions:

• Provide assistance and guidance in areas such as:
• Cooking, including shopping, food prep, following menus, and cleanup.
• Laundry, including inventory, washing, drying, folding, ironing, and storing clothing.
• Using durable medical equipment.
• Cleaning, including dusting, sweeping, mopping, and sanitizing all areas of the home.
• Personal care, including eating, bathing, grooming, and toileting.
• Providing transportation using agency or personal vehicles.
• Maintaining a positive attitude.
• Performing duties as caseload or shift varies.
• Engage positively with individuals at all times.
• Contribute to the development and implementation of Individual Plans, including documentation.
• Ensure health, safety, and wellbeing; report unsafe conditions or injuries.
• Perform delegated nursing and health-related tasks as assigned.
• Arrive on time; be flexible for holidays and shifts outside normal hours.
• Demonstrate professionalism, teamwork, and effective communication.
• Lead and communicate effectively; be dependable and collaborative.
• Follow agency policies and serve as a role model.
• Perform other duties as assigned.

Required Competencies and Credentials:

• Must be 18 or older.
• High school diploma or equivalent.
• Ability to follow instructions.
• Proficiency in English.
• Maintain certifications in Medication Administration, First Aid, CPR, PMT, and others as required.
• Valid Ohio Driver’s License, good driving record, and proper insurance.

Location: As assigned

Travel: Meetings, site visits, outings, and training outside the agency.

Reports to: Site Manager

FLSA Status: Non-Exempt; Full-time (35-40 hrs/week) or Part-time (<35 hrs/week)

Supervises: N/A

Works with: Individuals, families, professionals, medical staff, support staff, and maintenance personnel.

Physical Demands/Work Environment: The role requires standing, walking, sitting, driving, reaching, talking, and hearing. Occasionally lifting/moving up to 50 pounds. The environment is usually moderate noise. PPE may be required as per safety protocols.
