Job summary
A community support organization in Toronto is seeking a Temporary Support Worker 2 for a six-month position. The role involves identifying the needs of individuals, providing supports consistent with their life plans, and delivering case management focused on personal care and emotional support. Candidates must have a post-secondary education in Social Services or Developmental Services, or relevant experience, with knowledge of healthcare procedures preferred. A valid driver's license is an asset. This role has a workload of 37.5 hours per week and plays a crucial part in enhancing community living capacities.

- Knowledge of health‑care procedures and mechanical equipment (wheelchairs, lifts, G‑tubes and other specialized equipment);
- Post‑secondary education in a non‑related field requires at least one year of related work experience;
- Successful completion of internal legislated and mandated training;
- Valid driver’s license is an asset.
Identifies the needs of individuals supported and delivers supports and programs consistent with life plans to enhance their capacity to function in all aspects of community living.
- Under the direction of a program supervisor, identify program needs of individuals supported, participate in the development and implementation of life plans, and assist in monitoring achievement of personal outcomes.
- Provide case management to individuals on caseload, including personal and physical care, medication administration, crisis response, advocacy, and liaison with family, medical, and other professionals and service providers.
- Devising and providing life skills training and emotional support to individuals on a one‑on‑one or group basis and referring to professionals when necessary.
- Maintain documentation, monitor finances, ensure an appropriate standard of living, and assist with locating housing for individuals supported.
- Provide direction to part‑time staff as required.
