A regional health organization in Northeastern Ontario seeks a part-time Care Coordinator to assess patient care needs and coordinate services. The successful candidate will have experience in community health and must be a registered member of a relevant regulatory body. Responsibilities include developing care plans and working collaboratively with patients and healthcare providers to ensure high-quality patient-centered care. The role offers an hourly wage ranging from $41.29 to $51.13, with a start date of February 2, 2026.

Position: Care Coordinator RN, PT, OT or SW #173-25 Region: Algoma
Care Coordinator (Part‑Time, Startdate: February 2, 2026; Location: Sault Ste. Marie). You will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health‑care practitioners. Whether you work in our office, in the community, or a health‑care facility, you will play a lead role in delivering connected, accessible, patient‑centred care supported by a collaborative team of over 9,000 regulated health‑care professionals.

Salary: Hourly wage of $41.29 to $51.13
