As part of the Business Excellence & Execution Team, the Bilingual Field Nurse Case Manager (FNCM) is responsible for implementing, managing, and executing programs within the Patient Support Program(s). The FNCM aims to optimize and accelerate positive patient and program outcomes, supporting cross-functional teams.
Acting as the Single Point of Contact (SPOC) for patients, HCPs, and extended health care teams, the FNCM manages daily program activities, supports Healthcare Professionals (HCPs) and other stakeholders in the field, manages patient dossiers, and monitors metrics and KPIs.
Responsibilities include :
- Assist the PSP Lead in clinic and remote management, document coordination, and navigation services related to a program within the PSP operations.
- Provide bilingual support (English & French) for patients and HCPs.
- Monitor and report KPIs, offering insights for continuous improvement.
- Coordinate referral paperwork and assist with insurance coverage requests.
- Offer in-clinic support and follow-ups.
- Maintain accurate patient records and communicate effectively to address treatment concerns.
- Monitor patient progress and update the healthcare team regularly.
- Ensure ethical interactions and compliance with regulations.
- Participate in audits to uphold high standards.
- Provide feedback based on field observations and customer feedback.
- Collaborate with team members as per guidelines.
- Attend training sessions as required.
- Develop and implement program strategies and innovative solutions, including CRM and reporting improvements.
- Assist medical teams in managing HCP requests efficiently.
- Experience with Patient Support Programs, especially in Cardiology or Internal Medicine.
- Knowledge of the Canadian healthcare system.
- Customer service experience, preferably in a call center environment.
- Experience in clinical or hospital settings is an asset.
Qualifications :
- College or university degree.
- Nursing license in good standing, meeting provincial standards.
- At least 5 years of experience in the pharmaceutical, biopharmaceutical, or healthcare industry.
- Experience with Patient Support Programs.
- Fluency in English and French, both written and spoken.
- Experience with lipid-lowering therapy is an asset.
Working conditions and requirements :
- Mobility to work in an office environment with standard equipment.
- Ability to lift up to 30 pounds.
- Attention to detail with mental stamina to handle interruptions.
- Ability to travel up to 100%, including weekends and overnight stays, and work extended hours as needed.
