Job summary
A healthcare organization is seeking a Clinical Pharmacist for a remote position. Responsibilities include conducting telephonic consultations, documenting care, and using prescription claims data to provide clinical recommendations. Candidates must have a current pharmacist license and at least 1 year of remote working experience. Excellent knowledge of disease state management and proficiency in Microsoft applications are essential.

- Current and unrestricted Pharmacist license in the state of residence.
- 1+ years of experience working remotely/independently.
- Solid knowledge of disease state management guidelines and MTM services.
- Intermediate proficiency with Microsoft Word, Outlook, and Excel.

- Document care delivered and communicate information to primary care providers.
- Utilize prescription claims data to generate clinical recommendations.
