Job summary
A healthcare provider in Ontario is seeking a Registered Nurse to join their team. The role involves conducting home visits, facilitating care plans, and ensuring quality healthcare delivery. Ideal candidates will have a valid nursing license and at least three years of clinical experience. This full-time position requires reliable transportation and offers competitive benefits including a compensation package and generous PTO.

- Must be a licensed nurse. Registered nurse preferred.

- Experience in community settings preferred.
Responsibilities
- Facilitate receiving all medical records from the patient’s primary provider.
- Initiate a Care Management Plan of Care, if the patient is eligible.
- Communicate and coordinate care.
