Your Opportunity:

Located in the picturesque Foothills of Alberta and in close proximity to the Rocky Mountains, Pincher Creek Home Care has an opportunity for a dynamic and enthusiastic individual to join our team. Reporting to the Seniors Health Area Manager, the Registered Nurse (RN) Case Manager is responsible for collaborating and organizing comprehensive health care services for clients in a variety of settings. The RN Case Manager has experience in the role of advocate and can balance the needs of the individual, family, and community within organizational and legislative parameters. The Pincher Creek Home Care Team is knowledgeable with a strong commitment to quality patient care. The RN Case Manager has the ability to develop creative approaches to client care and demonstrates sound critical thinking and ethical decision making. The RN Case Manager demonstrates a patient and family centered approach with all client interactions.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:

Application of Comprehensive assessment, case management, and intervention techniques. Promoting Current trends, issues and practices in community care. Partnering with Community resources. Implementation of Current best practices in case management processes. Understanding and application of Quality Improvement theory and methodologies. Conflict resolution and crisis intervention. Leadership. Time management and organization. Negotiating skills with clients, caregivers and families. Family assessment skills. Effective oral and written communication. Working independently and as a member of a team. Working In a fast paced, ever-changing environment. Working with people in health care environment with professionalism and respect. Positive approach and good listening skills in dealing with challenging situations with empathy and sincerity. Knowledge of Guidelines for Assignment of Health Care Procedures by Health Care Aides. Ability to perform all assigned tasks and direct clinical care including but not limited to: Constant adjustment of position (sit/stand) throughout the shift. Physical exertion including bending/walking/sitting/ or standing for long periods of time. Keyboarding/mousing documentation for prolonged periods of time. Ability to be on call as required. Ability to perform all duties in accordance with related legislation.

Preferred Qualifications:

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Preference given to candidates with Recent Home Care Case Management experience (1-3 Years). Recent clinical experience in psychiatric, geriatric and palliative nursing preferred. Knowledge of and skills in applying Home Care Nursing Practice Standards.
