Reporting to the Provincial Manager - Intake Mental Health and Addictions Program, the Patient Access Navigator facilitates intake for MHA services via a centralized provincial telephone service. Responsibilities include screening, risk assessment, clinical triage, supportive counselling, psychoeducation, and resource sharing. The role involves building relationships, problem-solving, and supporting clients in navigating the healthcare system, reducing barriers to treatment, and liaising with community resources to ensure optimal care.

• Bachelor's degree in Social Work (required)
• Registration with Nova Scotia College of Social Workers (required)
• Minimum three years' experience in Mental Health and Addictions within healthcare
• Experience working with individuals with mental health/substance use concerns
• Strong motivational interviewing and engagement skills
• Proficiency with screening and triaging tools
• Knowledge of harm reduction and stages of change theories
• Awareness of community resources and development methods
• Knowledge of recovery-oriented practice and patient/family-centered care
• Non-Violent Crisis Intervention and Mental Health First Aid certifications are assets or willingness to obtain
• Working knowledge of DSM
• Ability to de-escalate situations and provide crisis support
• Cultural sensitivity and competency
• Additional language skills, especially French, are assets

Hourly rate: $36.58 - $47.35
