Salary Details: The salary range for this position is CAD $49.20/Hr. - CAD $63.47/Hr.

Practices in accordance with the standards of professional practice and code of ethics as outlined by the British Columbia College of Nurses and Midwives (BCCNM) as a registered nurse, within a client and family-centered care model and the organization’s vision and values. Facilitates patient hospitalization across the care continuum from admission through discharge. Coordinates with the interprofessional team to ensure effective patient care and timely discharge plans. Conducts comprehensive nursing assessments, guides the team on appropriate care levels, and develops sustainable discharge plans. Addresses barriers and provides leadership to achieve optimal outcomes, including securing funding and approvals for services. Develops policies, procedures, and standards to support care management and discharge planning. Uses data to improve practice, measure progress, and ensure patient-focused, high-quality care. Participates in committees, continuing education, and organizational initiatives.

Education & Experience: Current practicing registration as a Registered Nurse with BCCNM. At least three (3) years of recent clinical nursing experience, including one (1) year in case management or discharge planning, or an equivalent combination of education, training, and experience.

• Comprehensive knowledge of nursing theory and practice within a patient/family-centered care model across the continuum.
• Knowledge of BCCNM standards for nursing practice.
• Understanding of evidence-based nursing practice for various patient groups and ability to apply this knowledge in case management.
• Knowledge of pathophysiology, pharmacology, and ability to plan safe discharges.
• Understanding of medical appropriateness criteria, resource utilization, and transition/discharge planning.
• Knowledge of disease processes and potential long-term complications.
• Familiarity with other healthcare disciplines and community resources.
• Skills in patient assessment, observation, and monitoring.
• Ability to plan, organize, prioritize, analyze situations, problem-solve, and negotiate resolutions.
• Leadership skills, ability to work independently and in teams, and effective communication skills.
• CPR skills and computer proficiency in client care systems.
• Physical ability to perform job duties.

The hours and work area may change based on operational needs. All healthcare workers in publicly-funded facilities must report their vaccination history for communicable disease management. Applicants will need to provide this information during onboarding.
