Job summary
A community support organization in Hamilton is seeking Direct Support Facilitators for group living settings. The role involves providing assistance and support to clients, developing personalized support plans, and ensuring safety and wellness. Ideal candidates will have a Social Sciences degree and relevant certifications, along with experience working with diverse client groups. This position offers the opportunity to positively impact the lives of individuals with various needs in a collaborative environment.
Qualifications
- A minimum of 2 years experience working with adults with developmental disabilities.
- Education in Social Sciences or equivalent.
- Valid driver's license and certifications.
Responsibilities
- Provide support and assistance in group living settings.
- Develop person-directed plans for clients.
- Ensure safety and wellness of clients and coworkers.
Skills
Education
Direct Support Facilitators – Group Living provide support and assistance to clients within our group living settings, delivering social, recreational and learning based activities within the community.
Week 1: Saturday (3pm -11pm)
Week 2: Sunday (3pm-11pm)
- DSW, SSW, CYW, or Social Sciences degree or diploma in a related field. The above education with a DSW Certificate is preferred.
- Two (2) years of recent experience working with adults with a developmental disability, transitional aged youth, addictions and/or mental health preferred.
- Experience building an effective professional rapport with clients by focusing on respect, dignity, inclusion, and maintaining personal boundaries.
- Experience developing Person Directed Plans/Individual Support Plans with consideration for skill development, recreation, independent living skills, employment, health and wellness, and relationship building.
- Ensure the safety, security and wellness of clients, co-workers, and visitors.
- Displays conduct in the community that enhances the reputation of clients and Community Living Hamilton.
- Valid NVCI Certification.
- Valid First Aid/CPR Certification.
- A valid driver’s license.
- Fluency in a 2nd language.
- Experience with diverse client groups.
- Training/Education in Mental Health.
- Training/Education in ASD.
- Additional job-related training/ certifications.
