Job summary

A local health services provider in Northeastern Manitoulin and the Islands is seeking a Personal Support Worker on a temporary full-time contract to deliver personal support services and work with community health programs. This role requires cultural respect and communication skills, with a focus on promoting client well-being and independence. Candidates must hold a Personal Support Worker Certificate and have relevant community work experience. Flexible working hours are required.

Qualifications

  • Minimum of two (2) years community work experience required.
  • Current and clear CPIC.

Responsibilities

  • Provide personal support services based on the client’s care plan.
  • Assist with Meals on Wheels and provide Adult Day programming.
  • Work collaboratively with health care providers.

Skills

Knowledgeable and respectful of Anishinaabe customs, culture and language
Excellent verbal and written communication skills

Education

Personal Support Worker Certificate from a recognized college

Job description

Temporary Full-time contract (3 months)

Salary range: $22.82/hr – $27.27/hr

The Personal Support Worker is responsible for providing care that contributes to the quality of life of individuals who live at home by promoting their independence, dignity, social, emotional and physical well-being, mobility, personal appearance, comfort and safety so that they may remain and participate within their community.

The successful applicant must be willing to work flexible hours (possible evenings and/or weekends) and may provide services to all five First Nation communities (Aundeck Omni Kaning, Sheguiandah, Sheshegwaning, Whitefish River and Zhiibaahaasing).

OBJECTIVES:
  • Provide personal support services based on the client’s care plan or home management needs.
  • Provide assistance to the Home Care and Community Health Programs with the delivery of regular Meals on Wheels, provide Adult Day programming and may assist/provide generic transportation services for own client caseload, wellness visits and social safety.
  • Provide respite services working flexible hours.
  • Work collaboratively with health care providers in providing holistic care that focuses on achieving and maintaining optimal health.
  • Required documentation in Electronic Medical Records and regular check-ins with supervisor.
  • May assume additional duties as assigned by immediate supervisor.
