The Home Care Specialist will provide care coordination, case management, and system navigation services to clients enrolled in the, Hospital to Home and OHT‑lead programs/projects and other integrated care programs. This role plays a key part in supporting clients through their transition from hospital to home and continues to provide case management throughout their participation in the program. Acting as a liaison between home care clients, hospital partners, primary care and community care teams, the Home Care Specialist ensures continuity of care and alignment with program objectives. The position also fosters strong collaboration with program funders to enable smooth, client‑centered transitions, while contributing to quality improvement efforts aimed at optimizing patient flow, care coordination, and service delivery within the programs.

The Talent Acquisition Specialist (TAS) is key in sourcing, recruiting, and selecting top talent for nursing (RN/RPN) roles within the organization, recruiting, and selecting top talent for diverse roles within the organization. This dynamic position involves not only identifying and engaging potential candidates but also ensuring a smooth onboarding experience through comprehensive new hire orientation.
The TAS collaborates closely with managers, supervisors, and cross‑functional teams to develop and implement innovative recruitment strategies and retention initiatives with a focus on new hires. They leverage various tools and platforms to attract high‑caliber professionals and actively participate in career fairs, networking events, and industry conferences.
Moreover, the TAS monitors hiring trends and stays updated on best practices to continually improve the recruitment process. By fostering strong relationships and maintaining an exceptional candidate experience, the TAS plays a crucial role in building a talented and dedicated workforce that drives the success of the organization.
Full‑cycle recruitment
Consults with Managers, Supervisors, or Directors to determine recruitment needs and identify hiring criteria.
Identifies and implements innovative and effective recruitment and retention strategies and designs attractive job postings.
Plans, coordinates and implements all recruitment activities including posting jobs, sourcing candidates, reviewing resumes, conducting phones screens and interviews, executing background/reference checks, creating offer letters and/or contracts, and onboarding new hires.
Ensures that recruitment practices align with VHA’s DEI strategy, VHA recruitment policies, and collective agreements for unionised roles.
Keeps accurate records of information regarding applicants and recruitment
Utilizes Workday and GoldCare to maintain hiring records and documentation
Collaborates with recruitment agencies, job developers, and employment centres to identify talent for the organization.
Ensures that background checks are accurately completed upon hire as per VHA’s policies.
Ensures that candidate files are accurately updated throughout the recruitment process in the Applicant Tracking System.
Orientation & Onboarding
Delivers initial orientation training for new hires
Schedules Physical Readiness Test and assists with coordinating Field Training
Delivers mask fit testing for new hires as required
Makes recommendations and enhances orientation program.
Attends departmental and companywide meetings and participates with internal and external Committees.
HR Planning, Administration and Programs
Assists in identifying hiring needs based on input from all departments and future initiatives.
Monitors internal activities and external information to identify trends and impact on the organization.
Investigates unique recruitment/hiring strategies and makes recommendations for use at VHA.
Coordinates information and gathers statistics to assist with workforce planning.
Maintains records and statistics for HR activities and projects.
Writes correspondence, reports and other support for HR Department.
Contributes and participates in employee relations and recognition initiatives.
Ensures that employee and service provider files are accurately created and maintained in the recruitment and HRIS systems for new hires.
Team and Service Effectiveness
Participates in team meetings with a commitment to contribution and service development.
Takes part in project teams as appropriate.
Provides statistics and information for reports and decision making.
Supports team decisions and strategies.
Suggest recruitment best practices and participate in their implementation
Shares in handling workload for positive results.
