The HIM Professional is responsible for ensuring the delivery of complete and accurate clinical records in full compliance with all applicable provincial legislation. This role involves monitoring Mental Health Act forms and managing health record information, including liaising directly with clinical staff to facilitate this mandate. Ensuring the confidentiality of patient information is maintained in accordance with all ethical and legal requirements.
Duties
- Accountability #1: Quantitative Analysis of Clinical Records for Completeness
- Performs analysis of clinical records for completeness, accuracy, quality, validity, and reliability, ensuring compliance with hospital accreditation standards, statutory, and legal requirements.
- Verifies outpatient chart openings and re-openings, flags errors, and ensures resolution.
- Assists secretarial staff with Meditech applications and inputs/edits chart deficiencies in the Meditech tracking system.
- Maintains Meditech Borrower files and monitors physician and discipline compliance with chart deficiency policies.
- Reports non-compliance and maintains deficiency letters.
- Assists clinical staff with inquiries regarding the chart deficiency process and completion of deficiencies.
- Coordinates hearings related to the Office of Consent and Capacity Board and assists relevant hospital staff.
- Accountability #2: Completion of RAI tool
- Resolves diagnosis discrepancies by consulting with physicians and assists with data completion in the RAI tool.
- Accountability #3: Monitoring of Mental Health Act Forms
- Reviews forms for validity, prepares reports, and manages data entry related to legal status and funding requests.
- Accountability #4: Registration
- Updates inpatient/outpatient data in the Admissions/Discharges/Transfers system, manages discharge processes, and ensures chart completion.
- Accountability #5: Release of Patient Information
- Handles requests for patient information, coordinates access with treatment teams, and manages documentation and logging.
- Accountability #6: Other Professional Responsibilities
- Prepares and verifies scanned documents, trains students, participates in policy development, and tests patient information systems.
- Accountability #7: Other Duties
- Adheres to safety and anti-discrimination policies, participates in committees, and performs additional duties as assigned.
Qualifications
- Community College Diploma in Health Information Management
- Certification and good standing with the Canadian Health Information Management Association (CHIMA)
- Experience with ICD-10 coding; Meditech experience is an asset
- Intermediate skills in Microsoft Office; high typing accuracy and speed
- Strong organizational, interpersonal, and communication skills
- Attention to detail, ability to work under pressure, and excellent attendance
- Proficiency in English; bilingualism is an asset
