Care Coordinator – Patient Care Management

Our client is looking for Care Coordinators to assist patients and clinicians in the projects we support. The Care Coordinator would actively identify and onboard complex patients, facilitating the ongoing care of these patients in an appropriate care setting, in collaboration with the consortium multidisciplinary team, patient, family, and local community services. This requires building and maintaining excellent relationships with all levels of healthcare providers and patients. The Care Coordinator is effectively responsible for scheduling, organizing, and managing all aspects of a patient’s healthcare maintenance and treatment. Duties include helping patients complete paperwork, communicating with the healthcare team about a patient’s treatment plan, and educating patients about resources and options for managing their health.

Job Description

WHAT WE NEED YOU TO DO:

  • Perform health assessments on patients to identify individual patient risk factors
  • Propose interventions as pre-defined to influence individual patient risk
  • Actively participate in the management of complex patients by building good relationships with the patient and their family
  • Monitor patient progress through ongoing patient and clinician interactions
  • Promote behaviour change to ensure greater treatment compliance and condition management
  • Coordinate the integration of care, including social services
  • Assist in navigating additional benefits
  • Arbitrate and resolve conflicts and interface with problems that arise in patient care
  • Coordinate regular liaising and communication with the care team (including clinicians, hospital staff, family members, counsellors, and admin staff)
  • Assist in the documentation of clinical information
  • Maintain relationships between external treating providers, the consortium’s multidisciplinary team, and the management team
  • Build teams in the healthcare industry – inspiring teamwork at all levels to ensure optimal outcomes for the patient and the team
  • Understand the team’s operational processes to improve clinical process development
  • Engage with clinical data to understand issues and provide clinical input into a broad range of development work, including creation of templates and processes to improve team workflow and patient outcomes
  • Actively contribute to improvements to ensure the success of the bigger team
Qualifications and experience required
  • Clinical Associate or Registered Nurse: Bachelor of Clinical Medical Practice / Degree in Nursing; 5 years of industry experience with a minimum of 2 or more of the following:
    • 3 or more years of clinical experience
    • Care Management or similar experience
    • Multidisciplinary team experience
