A major healthcare provider in Singapore is seeking a dedicated individual to provide efficient, integrated health and social care. You will monitor residents' wellbeing, coordinate care with various stakeholders, and assist with community outreach. The ideal candidate has a relevant diploma and at least 2 years of experience in the healthcare sector. Strong communication skills are essential for this position.
Qualificações
- Minimally 2 years of relevant experience.
- Good verbal and written communication skills.
- Operational experience in health and community care sectors preferred.
Responsabilidades
- Monitor residents' wellbeing and care plan through regular check-ins.
- Coordinate health and social care by liaising with various stakeholders.
- Implement changes to care plans in consultation with staff.
- Maintain updated documentation of care plans.
- Assist in screening residents for health and social care needs.
- Share information on suitable community resources.
- Support community outreach and engagement efforts.

Your role is to provide efficient and effective integrated person-centred health and social care to residents residing in SingHealth region. You will support Senior Associate Wellbeing Coordinators (SAWBCs) and WBCs in identifying and carrying out the interventions from the residents’ care plan. You will serve as a touchpoint to SingHealth residents and patients, and journey with them towards better health.
- Monitor residents' wellbeing and care plan through regular check-ins
- Coordinate health and social care by liaising with various stakeholders within the integrated community care team
- Implement changes to care plans, developed with residents, in consultation with SAWBCs (Sr Associate Wellbeing Coordinator) and WBCs (Wellbeing Coordinator)
- Maintain updated documentation of care plans, post-intervention follow-ups, and engagements with residents and/or caregivers
- Assist in screening residents for health and/or social care needs, such as administering basic tests and questionnaires, and assessing home environment and financial situation
- Share information on suitable community resources and services
- Support the team with community outreach and engagement efforts
- Local Diploma in healthcare or non-healthcare related studies
- Higher NITEC / NITEC / GCE N/O/A Level with minimally 2 years of relevant experience
- Good verbal and written communication skills
- Operational experience in and knowledge of health, social and community care sectors preferred
