Summary

Serves as a pharmaceutical caregiver to specific patient population by efficiently designing, recommending, activating, monitoring, and evaluating patient-specific pharmacotherapy to ensure optimal outcomes under the direct supervision of the respective preceptor. Ensures implementation of the pharmaceutical care plan by assuring the accurate and timely distribution of medications to patients, Serves as a clinical resource for other pharmacy personnel, medical and nursing staff, as well as other healthcare professionals.

Essential Responsibilities and Duties
  1. Reviews,evaluates, and processes medication orders for demographic data (e.g., gender,age, and weight), appropriateness of therapy, dose, frequency, route, dosageform, interactions, allergies, related laboratory values, and possible druginteractions.
  2. Providesdirect patient care by actively participating in rounds, reviewing patientcharts, and developing and implementing patient - specific pharmacotherapy planto ensure optimal outcomes.
  3. Ensuresimplementation of therapeutic regimens and monitoring plans (care plans) bytaking appropriate follow-up actions.
  4. Documentsinterventions and recommendations/clarrifcations in the electronic HealthInformation System as applicable.
  5. Detects,analyses, and reports adverse drug reactions.
  6. Designsexecute or participate in a research project that may include data collection,statical analysis, and write research papers.
  7. Participates/assistsin conducting performance/quality improvement projects.
  8. Activelyparticipating in conducting medication use evaluation (MUE).
  9. Providesinformation and education resource for patients, students, colleagues, medicaland nursingstaff, and other Healthcare professionals.
  10. Participatesin self and others' education, training and development.
  11. Assuresadherence to the departmental policies and procedures.
  12. Followsall Hospital’s related Policies and Procedures.
  13. Participatesin self and others' education, training and development, as applicable.
Education

Pharm.D./Bachelor’sDegree in Pharmacy is required

Experience Required

Completionof Saudi Diploma in Clinical Pharmacy residency program is required.
