An exciting opportunity has arisen for a dynamic staff nurse to join First Community Health and Care working with the Responsive Services Team.

Responsive Services consists of the Urgent Community Response Team, Intermediate Care Team, Virtual Ward, and Referral Hub.

This role involves collaboration with Primary Care and Secondary Care colleagues, Urgent Community Response, Intermediate Care (Responsive Services), Community Nursing, and other Specialist Healthcare Teams across First Community, statutory, and voluntary agencies to deliver excellent patient care for those who are acutely unwell and require support to avoid hospital admission. It includes providing practical care, education, and training for patients and carers admitted to responsive services.

You will complete assessments in patients' homes and undertake treatments such as IV medications, wound assessment, and management. You will work closely with the multidisciplinary team to promote patient independence, enabling them to remain in their own homes.

The candidate must be adaptable to change and capable of working in a busy environment. Excellent communication skills are essential for liaising with health and social care partners in community assessments and management.

Applicants must be able to drive, possess a suitable car for business purposes, and hold a clean driving license.

The post holder will work under the direction of the virtual ward charge nurse and collaborate with Primary Care, Secondary Care, Urgent Community Response, Intermediate Care, Community Nursing, and other Healthcare Teams to provide high standards of care to acutely unwell patients, helping them avoid hospital admission. • Competence in intravenous additives, venipuncture, ECG analysis, urinary catheterization, and bowel management

• Current NMC registration
• Registered First Level Nurse
• Degree or diploma level education with Mentorship, ENB 998, practice assessor, or equivalent
• Evidence of ongoing professional development

This role requires a DBS check due to the Rehabilitation of Offenders Act (Exceptions Order) 1975.

Salary: £29,970 to £36,483 per year + 5% HCAS, pro rata.
