Job Overview

We are seeking a dynamic and compassionate Advanced Clinical Practitioner (ACP) to join our innovative Virtual Frailty Ward team. This is an exciting opportunity to deliver high-quality, patient‑centred care to older adults with frailty, enabling them to receive hospital‑level support in the comfort of their own homes.

Key Responsibilities
  • Assess, manage, and coordinate care for patients with complex needs via virtual assessment, including community assessments when necessary.
  • Lead clinical decision‑making and provide expert management of acute and chronic conditions.
  • Collaborate with consultants, nurses, therapists, pharmacists, and social care teams to deliver integrated care.
  • Monitor patients’ progress using remote technologies and respond rapidly to changes in condition.
  • Support advanced care planning, including discussions around goals of care and end‑of‑life planning where appropriate.
  • Contribute to service development, audit, and education to ensure high standards of care and continuous improvement.
Qualifications & Criteria
  • Registered with NMC, HCPC, or relevant professional body.
  • Master’s level qualification (or equivalent experience) in Advanced Clinical Practice.
  • Independent non‑medical prescriber qualification.
