A busy town centre practice in Uxbridge is seeking a part-time Practice Nurse to manage diabetes disease and conduct clinics for registered patients. Responsibilities include administering vaccinations, ECG tracings, and maintaining patient records. The ideal candidate is a Registered General Nurse with experience in chronic disease management and proficient with the EMIS clinical system. This role offers flexible hours and competitive salary based on experience.
We are recruiting a practice nurse to replace a leaver. The successful candidate should have an interest in diabetes as they will have responsibility for diabetes disease management and the annual reviews and QoF. Also assist in chronic disease management clinics.Salary dependant upon experience.

We are a large, busy town centre practice with 16,700 patients.The team currently consists of 8 GPs, 2 ANPs, 3 nurses, 1 HCAs and a pharmacist.The reception and admin function currently has 17 staff.We are part of the Synergy PCN delivering services to a large part of Uxbridge. We have the support of PCN staff in the form of a pharmacist, pharmacy technicians, physiotherapist, social prescriber, mental health nurse and community matron with CCT team.
REPORTS TOThe Lead Nurse (Clinically)The Practice Manager (Administratively)GENERAL PURPOSETo undertake daily clinics as arranged for the treatment of registered patients. To include Health Promotion as necessary. All clinics are open to variation as necessary according to current legislation and/or patient care. Practice policy states all protocols and guidelines available are to be followed.DUTIES TO INCLUDE: ECG tracings Immunizations and Vaccinations adults and children, to include the seasonal flu campaign, Intramuscular, intradermal and subcutaneous Injections as authorised by the patients GP Knowledge and application of emergency procedure for anaphylactic shock Assisting with minor operations Cervical smears in line with national screening policy Maintain the computerised patients clinical database and manual medical records - contemporaneous notes at all times New Patient health checks NHS Health checks Attendance where practical at the weekly PHCT meetings or at least one per month Wound care (as referred by GPs, colleagues or 2 care) - referred from GP sessions Tissue viability - wound and ulcer care, Contraception injections and routine checks Any other duties as may be reasonably required by employer. Appropriate referral to other agencies when appropriate
