A leading healthcare provider in Leeds seeks a Pharmacy Technician to support clinical pharmacists and improve prescribing practices across its care network. Candidates should be experienced, proactive, and possess excellent communication skills. Main duties of the job

Organisational Expectations:

You will be working towards the delivery of theoutcomes outlined in the Primary Care Network Contract Enhanced ServiceSpecification.

The post holder will be employed by South East Leeds GPGroup, but will be deployed and managed day to day from within the Middleton& Hunslet Primary Care Network

Pharmacy technicians play an important role,complementing clinical pharmacists, community pharmacists and other membersof the PCN multi-disciplinary team. Pharmacy technicians are different toclinical pharmacists as they are not able to prescribe or make clinicaldecisions, instead working under supervision to ensure effective andefficient use of medicines.

Pharmacy technicians core role responsibilities willcover clinical, and technical and administrative categories.The purpose of the role is to lead improvements tomaximise safe, cost-effective best practice in prescribing to improve thequality of patient care. The post holder will help patients to get the bestfrom their medicines by switching medications to agreed and approved protocols,improving repeat prescribing processes in General Practice, including promotionof repeat dispensing and online ordering, minimising clinical risk and aimingto reduce wasted medicines.

About us: Middleton& Hunslet Primary Care Network

Primary CareNetworks (PCNs) form a key building block of the NHS long-term plan. Bringinggeneral practices together to work at scale has been a policy priority forsome years for a range of reasons, including improving the ability ofpractices to recruit and retain staff; to manage financial and estatespressures; to provide a wider range of services to patients and to integratewith the wider health and care system more easily. PCNs were formallyratified in July 2019.

Middleton andHunslet PCN is a large PCN with a patient list size of circa 33,000 patients.Of this, we currently provide clinical support to 10 Care Homes, with apopulation of around 360 beds. We have a significant workforce in place whichbrings wonderful opportunities for in-house multi-disciplinary team working. Wehave a well-developed multidisciplinary team including a Pharmacy Team,Occupational Therapists, Social Prescribers, a range of nursing/ HCA staffand more which means that the majority of any patient's needs can now beprovided on their own doorstep.

Middleton& Hunslet PCN also works collaboratively with Beeston PCN, forming BMH(Beeston, Middleton & Hunslet PCNs, we have roles working across bothensuring a continuity of care between. To note this role is for that onlyof Middleton and Hunslet PCN.

Job responsibilities

The purpose of the role is to lead improvements tomaximise safe, cost-effective best practice in prescribing to improve thequality of patient care. The post holder will help patients to get the bestfrom their medicines by switching medications to agreed and approvedprotocols, improving repeat prescribing processes in General Practice,including promotion of repeat dispensing and online ordering, minimisingclinical risk and aiming to reduce wasted medicines.

In addition, the post holder will be responsible forencouraging the development of better understanding of the principles ofmedicines optimisation throughout the practice teams and promoting goodpractice in line with therapeutic developments. This will involve assisting the PCN inachieving national requirements, NICE implementation and utilisation ofmedicines optimisation initiatives.

You will be working towards the delivery of theoutcomes outlined in the Primary Care Network Contract Enhanced ServiceSpecification

Main Tasks:

Clinical

Undertake patientfacing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients.

Carry out medicinesoptimisation tasks including effective medicine administration (e.g.,checking inhaler technique), supporting medication reviews and medicinesreconciliation. Where required, utilise consultation skills to work inpartnership with patients to ensure they use their medicines effectively.

As determined by thePCN, support medication reviews and medicines reconciliation for new carehome patients and synchronising medicines for patient transfers between caresettings, linking with local community pharmacies

Support the ClinicalPharmacist in Structured Medication Reviews (SMR) e.g., organise necessarymonitoring tests prior to SMR

Provide expertise toaddress both the public health and social care needs of patients, includinglifestyle advice, service information, and help in tackling local healthinequalities.

Manage shared careprotocols and liaise with Clinical Pharmacists for more complex patients.

Support initiatives for antimicrobialstewardship to reduce inappropriate antibiotic prescribing locally.

Technical & Administrative

Support the PCN multi-disciplinary team toensure efficient medicines optimisation processes are being followed.

Implement efficient ordering and returnprocesses and reducing medication wastage.

Provide training and support on the legal,safe and secure handling of medicines, including the implementation of theElectronic Prescription Service (EPS).

Promotion of Electronic Repeat Dispensing(eRD) and online ordering

Develop relationships with other pharmacyprofessionals and members of the multi-disciplinary team to supportintegration across health and social care including primary care, community pharmacy,secondary care and mental health.

Support practice reception teams in streaminggeneral prescription requests, to allow GPs and clinical pharmacists toreview the more clinically complex requests.

Support the implementation of nationalprescribing policies and guidance within GP practices, care homes and otherprimary care settings.

Support the PCN to deliver on the QualityImprovement Plans, QOF and locally commissioned enhanced services

Support the PCN in reviewing and developingpractice policies for CQC requirements

Health and Safety - Thepost-holder will assist in promoting and maintaining their own and othershealth, safety and security as defined in our Health & Safety Policy, toinclude:

Using personal security systems within theworkplace according to PCN guidelines.

Identifying the risks involved in workactivities and undertaking such activities in a way that manages those risks.

Making effective use of training to updateknowledge and skills.

Using appropriate infection controlprocedures, including maintaining a tidy and safe way work area free fromhazards. Effective hand hygiene.

Provide advice on the correct and safemanagement of the specimen's process, including collection, labelling,handling, use of correct containers, storage and transport arrangements.

Correct use of personal protective equipment(PPE) in both routine and extraordinary circumstances.

Safe use of sharps, storage and disposal.

Recognise people's needs for alternativemethods of communication and respond accordingly.

Use developed communication, negotiation andconflict management skills recognising the need for alternative methods ofcommunication to overcome different levels of understanding, culturalbackground etc.

Managing Risk:

Manage and assess risk within the areas ofresponsibility, ensuring adequate measures are in place to protect staff andpatients. This includes the escalation of clinical concerns or appropriatereferrals where the management of a clinical situation requires it, or itfalls outside of clinical competence.

Monitor work areas and practices to ensurethey are safe and free from hazards, and conform to health, safety andsecurity legislation, policies, procedures and guidelines.

Apply infection control measures within thepractice according to local and national guidelines and in accordance withthe practice policy.
