Summary:
Job Purpose: The Clinical Pharmacist will provide expertise in clinical medicines optimization across the PCN, contributing to various aspects of patient care and system improvements. They will work within a multi-disciplinary team, conducting structured medication reviews, managing long-term conditions, and enhancing health in care homes, among other responsibilities. The role is pivotal in improving patient outcomes and operational efficiencies within general practice.
Main Responsibilities:
Patient-facing Clinical Medication Review: Conduct clinical medication reviews, implement necessary changes, and provide recommendations for patient care.
Medicines Quality Improvement: Undertake clinical audits, identify high-risk patient groups, and implement changes to enhance prescribing practices.
Medicines Safety: Implement changes based on MHRA alerts and other guidelines to ensure patient safety.
Leadership and Management: Contribute to leadership in medicines-related issues, demonstrate understanding of governance, and motivate self and team to achieve goals.
Management of Common/Minor Ailments: Manage patients with common/minor ailments, signpost to appropriate healthcare professionals, and provide patient-facing and telephone support.
Management of Medicines at Change of Care Setting: Reconcile medicines post-discharge, ensuring continuity of care for high-risk patient groups.
Medicine Information and Drug Monitoring: Provide medicine-related information to practice staff and patients, and ensure robust systems for drug monitoring.
Signposting and Repeat Prescribing: Refer patients to appropriate healthcare professionals, contribute to repeat prescribing processes, and ensure appropriate patient monitoring.
Service Development and Education: Contribute to service development, analyze medicines data, and participate in education and training activities.
Care Quality Commission and Public Health: Ensure compliance with CQC standards, support public health campaigns, and provide specialist knowledge on public health programs.
Special Working Conditions: The role may involve independent travel, contact with body fluids, and home visits.
Communication and Working Relationships: Collaborate with PCN Clinical Director, engage with multi-disciplinary teams, and foster strong relationships with stakeholders for patient benefit.
