Job Summary

Join our dynamic team as an Advanced Pharmacist in Anticoagulation and Thrombosis on a 1-year fixed-term contract. Collaborate with healthcare professionals, ensuring compassionate excellence and promoting medicines safety. This role involves split duties between the Anticoagulation Optimisation Support Service (AOSS) to primary care and the clinical anticoagulation role within Oxford University Hospitals NHS Foundation Trust.

Key Requirements:

Experience in anticoagulation with a proven record of multidisciplinary working.

Confidence and skills to excel in unfamiliar and challenging environments.

Excellent communication and organizational skills.

Responsibilities Include:

Reviewing patients on warfarin with poor anticoagulation control.

Leading on aspects of the inpatient role, such as VTE prevention or haemophilia.

Unifying and simplifying anticoagulation guidelines in primary care.

Providing clinical support for GPs and pharmacists, including Direct Oral Anticoagulants (DOACs).

Main Duties

Delivering pharmaceutical aspects of anticoagulation and thrombosis within AOSS.

Managing case referrals for medicines optimization from primary care and internal thrombosis MDT.

Reviewing patients on warfarin therapy with poor anticoagulation control.

Unifying anticoagulation guidelines in primary care.

Contributing to educational teaching and providing clinical support for GPs and pharmacists.

Providing email and telephone advice support for complex anticoagulation inquiries.

Measuring the impact of interventions made by AOSS.

Participating in anticoagulation clinics, primarily for patients on warfarin with poor control.

Being a member of the Medicines Safety Team.

Supporting the Pharmacy Education and Training Team for competency in anticoagulant-related roles.
