Job description
Overview

You will be part of an innovative recruitment group on an 18-month Newly Qualified Nurse (NQN) rotational programme supported by JUCD and the University of Derby. You are expected to remain on the programme for the full 18 months, after which you will be offered a Band 5 position within JUCD from one of the supporting organisations (DCHS, UHDB, CRH or DHCFT). It is an expectation that you will participate in, evaluate and engage with research throughout the 18 months. The rotational pathway will consist of three rotational workplaces for a duration of up to 6 months each. You will attend leadership and development workshops and complete the DCHS preceptorship programme. You will assess, plan, implement and evaluate individualized patient care using your professional judgement at all times, ensuring patient care is delivered efficiently and clinically effectively while maintaining the patient\'s safety, privacy and dignity.

You will provide advice and support to junior staff and other members of the multi-disciplinary team in delivering high-quality care. You will actively support District Nurses/Community Nursing Sisters/integrated community teams/acute sector staff/GP teams in managing the healthcare environment.

Responsibilities
  • This is a rotational post; environments will vary.
  • Support the team leads in day-to-day operational management of the team and patient caseload, ensuring appropriate delegation of care and workload, and high-quality outcomes for patients. This includes day-to-day coordination of other nursing team members, linking to key developments, and using the caseload management toolkit/System One/IT Systems and related processes.
  • Assess and implement treatment and develop care programmes for designated patients across the rotational area; provide feedback and escalation to meet patient needs through supervision/monitoring processes.
  • Work in close liaison with rotational team leads to take responsibility as a named nurse for patients on the caseload.
  • In the absence of the team lead, take delegated responsibility for caseload management to ensure continuity of care in accordance with the care programme, seeking support from other specialist practitioners/operational managers as required.
  • Work as part of an integrated team within the rotation, supporting other health professionals to provide a seamless and consistent nursing service.
  • Identify patient needs, order appropriate equipment in a timely manner, and provide advice, support and demonstration to patients/carers on equipment use; monitor and report the effectiveness of equipment and aids to maximise resource use for care packages.
  • Maintain and record contemporaneous accurate health records on SystmOne/IT systems in line with Caldicott and Trust policies. Update other systems (e.g., EMIS, Inform) as required. Communicate verbal information to relevant multi-agency/multi-disciplinary team members.
  • Assist the role of a clinical champion in an area of interest (e.g., Safe Care Champion, Pressure Ulcer Champion) and disseminate information learned.
  • Support placement of pre-registration Nurse Students, Trainee Nursing Associates and Return to Practice Students; contribute to the learning environment.
  • Be involved in the education and training of junior staff and other staff within care settings in area of expertise/competence.
  • Participate in an active evaluation process, clinical audits (national and local), and an evaluative research process.
  • Contribute to Public Health, Health Promotion and Self-Care initiatives under rotational lead direction to support well-being and independence for patients, relatives and carers.
  • Participate in routine screening for chronic disease management and record results for monitoring purposes to ensure compliance with National Service Frameworks.
  • Maintain up-to-date competence in line with the agreed competency framework; participate in clinical supervision/reflective practice as per Trust Policies and NICE guidance.
  • Play an active role in the Trust Staff Wellbeing agenda as a registered health professional (e.g., participate in staff flu immunisation sessions).
  • Attend required operational meetings; if unable to attend, ensure updates are received and actioned. Maintain confidentiality at all times and work within the Lone Working policy.
  • When working in the community, adapt daily work plans to reflect the agile nature of community work and maximise patient-facing clinical time.
  • Provide nursing care using current evidence to inform delivery and development of clinically effective, personalised care; coordinate assessment, planning, implementation and evaluation of care programmes to meet patient and carer needs.

Awareness and understanding of Public Health and Self-Care agendas, evidence of research-based/reflective practice, and knowledge of local and national priorities are essential. You must adhere to health and safety policies and report incidents via the Trust reporting system. You must be able to work across a 7-day pattern and travel as required.
