We arelooking for a dedicated and compassionate Practice Nurse to join our friendlymultidisciplinary team. In this role, you will deliver high quality, evidence basednursing care while working both independently and collaboratively withcolleagues across primary, community, and secondary care.

You will be involved in a wide range of clinical areas, including chronic diseasemanagement, immunisations, screening programmes, infection prevention, healthpromotion, and well man/woman services. You will also support our populationhealth approach, helping to reduce health inequalities and improve access forour patients.

Main duties of the job
  • Deliver evidence based nursing care across a wide range of clinical areas, including long term condition management, immunisations, cervical screening, wound care, health promotion, and infection prevention and control.
  • Undertake patient assessments, provide routine and urgent nursing care within scope of practice, and offer advice to patients and their families on managing their conditions.
  • Support population health approaches by contributing to public health campaigns, targeting health inequalities, maintaining disease registers, and using call/recall systems effectively.
  • Work collaboratively with the multidisciplinary team and external agencies to ensure coordinated, high quality patient care.
  • Participate in clinical audits, quality improvement initiatives, research projects, and the development or review of practice policies, protocols, and procedures.

Parkside Group Practice is a well established GP surgery located in South Croydon, providing compassionate, patient centred care to a diverse population of approximately 13,500 patients.

Our team is supportive, collaborative, and multidisciplinary, comprising

  • 4 GP Partners
  • 1 Paramedic
  • 2 Practice Nurses
  • A dedicated administration team of 18, including a Business Manager and two Operations Managers

Job responsibilities
  • Deliver evidence based nursing care across a wide range of clinical areas, including long term condition management, immunisations, cervical screening, wound care, health promotion, and infection prevention and control.
  • Undertake patient assessments, provide routine and urgent nursing care within scope of practice, and offer advice to patients and their families on managing their conditions.
  • Support population health approaches by contributing to public health campaigns, targeting health inequalities, maintaining disease registers, and using call/recall systems effectively.
  • Work collaboratively with the multidisciplinary team and external agencies to ensure coordinated, high quality patient care.
  • Participate in clinical audits, quality improvement initiatives, research projects, and the development or review of practice policies, protocols, and procedures.
  • Maintain accurate clinical records, comply with national and local guidelines, and uphold standards within areas such as safeguarding, clinical governance, and infection control.
  • Engage in ongoing training, clinical supervision, annual appraisal, and contribute to shared learning within the practice.
  • Support everyday operational needs, including chaperoning, assisting with clinical emergencies, ensuring a clean working environment, and helping with rota coordination when required.

Qualifications
  • Registered Nurse (Nursing and Midwifery Council)
  • Has evidence of working at the relevant level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
  • Mentor or teaching qualification.
Experience
  • Experience of working autonomously.
  • Experience of infection prevention and control measures.
  • Experience of managing elderly patients.
