Senior Clinical Fellow post in Paediatric Emergency Medicine

The post is based in the children's emergency department, where the post-holder is responsible for the care of all patients, under supervision of a consultant. The senior clinical fellow would be expected to direct the management of ill and injured children, participate in the resuscitation and stabilization of severely ill children, and assist in the management of severely injured children.

Applications are invited for a Senior Clinical Fellow post in Paediatric Emergency Medicine to join the Evelina London Children's Emergency Department team at St Thomas' Hospital. This post would be ideal for a doctor with a background in paediatrics or emergency medicine with experience working on a registrar rota, as well as for a doctor at the end of their training who wish to consolidate their skills in paediatric emergency medicine. The Trust also has one of the largest simulation centres in the UK with close ties to the Children's Emergency Department, running regular in situ simulations and multiple simulation‑centre paediatric and emergency medicine courses. This post would make a perfect OOPE placement with excellent opportunities in subspecialty training, teaching, simulation training and research.

The department offers weekly, protected teaching sessions in paediatric emergency medicine and hosts regional paediatric emergency medicine training days. Each of our junior doctors has a paediatric emergency medicine educational supervisor, helping make individual development plans to tailor training in particular skills and areas to their educational needs. The department is accredited for training in paediatric emergency medicine by both the Emergency Medicine and Paediatric Postgraduate Deaneries.

Job responsibilities

The post-holder will receive formal training through the postgraduate teaching programmes in both the Paediatric and the Emergency Departments. Each week the paediatric department has a one-hour grand round presentation and an X‑ray meeting. The emergency department has weekly two‑hour consultant‑led teaching covering a range of topics in emergency and general paediatrics. Paediatric resuscitation training scenarios are run on a regular basis and Paediatric ED specific teaching on the 3 other days.

During the Covid-19 period teaching is still featuring as usual, but mostly virtual apart from simulations and ED specific teaching, which are still running face‑to‑face. However, one advantage of this is there is now a bank of teaching which can be accessed remotely, and we are trying to be creative to still provide a good programme of learning. We have twice weekly ED specific teaching involving trainees and twice per month paediatric X‑ray ED specific teaching and Mini SIMs twice monthly.

Within the large paediatric and emergency departments there is great potential for the post-holder to undertake training in particular skills/areas as per his/her requirements. Such training can be organised on an individual basis.

Study leave, and funding is available to the post-holder, though the provision of leave remains at the discretion of the lead consultant. The post-holder's training will be supervised by both paediatric emergency department consultants, and head of service for A&E.

There are also further opportunities to rotate into a General Paediatric post after 6 months, subject to availability.

Research and audit

There is an active audit and research programme in the department.

Emphasis will be placed on research and audit activities and it is hoped that this post will provide excellent opportunities for the trainee to present or publish a scientific communication as well as provide a clinical service.

Rota arrangements:

The clinical fellow will participate in out‑of‑hours cover for the paediatric emergency department only. At present this is a full shift band 1A.

Teaching:

The post-holder will be expected to contribute as appropriate in the teaching of undergraduate and postgraduate students.

Clinical Governance:

All medical and dental staff are expected to take part in clinical governance activity, including clinical audit, clinical guideline and protocol development and clinical risk management. They will be expected to produce evidence of their contribution in these areas and their audit of their own clinical work as part of their appraisal.

Mutual Obligation to Monitor Hours:

There is a contractual obligation on employers to monitor working hours through robust local monitoring arrangements supported by national guidance, and on individual doctors to co‑operate with those monitoring arrangements.

Monitoring of Working Time

All posts and working patterns are under constant review in line with GMC guidelines for junior doctors.

Educational Programme:

Neither the London Deanery nor the Royal College accredits this post for postgraduate training. However, the post-holder will be expected to attend and contribute to educational activities e.g. departmental meetings, X‑ray meetings, pathology meetings, multidisciplinary meetings, journal clubs etc.

Appraisal:

All medical and dental staff are required to undertake appraisal.

Qualifications
  • GMC registration
  • MB BS (or equivalent)
  • APLS or similar paediatric emergency resuscitation course provider
  • Complete Foundation competencies or equivalent
  • BSc, BMedSci (or other intercalated degree), other degrees / qualifications
  • MRCPCH part 1, MRCP, MRCPCH, MCEM or equivalent
  • APLS Instructor or equivalent resuscitation provider training
  • Level 3 child protection training
CLINICAL EXPERIENCE
  • Previous paediatric or neonatal experience
  • Experience as middle grade in paediatrics or Emergency Medicine
  • Minimum 2 years as SHO in paediatrics and its specialties
  • Additional experience in one or more of other related specialties (paediatric medicine, paediatric intensive care medicine, paediatric surgery, anaesthesia, trauma & orthopaedics, plastic surgery)
CLINICAL SKILLS
  • Understanding of clinical audit and risk management
  • Experience in clinical procedures relevant to the post.
  • Child protection experience and skills
  • Experience in quality improvement work or service development
  • Evidence of contribution to clinical audit and risk management
