A prominent medical practice in Melton Mowbray is seeking an experienced Clinical Pharmacist. In this role, you will manage patient medication queries, conduct clinical reviews, and collaborate with healthcare teams. Latham House Medical Practiceis looking for an experienced Clinical Pharmacist to complement the existingteam of Pharmacists and Technicians.
Working alongside our current PharmacyTeam the post-holder will quickly become an integral member of the Practice andenjoy a diverse range of duties and responsibilities including:
Resolving medicinesrelated queries with both healthcare professionals and patients.
Further develop thePractices strong working relationships with care homes and community nursingsupporting the management of patients.
Conductingmedicines/polypharmacy reviews either in a face-to-face clinical setting or by telephone.
Developing their clinicalskills in a chosen area of expertise.
Contributing to andsupporting the Audits and Quality Improvement Programme to ensure prescribingis aligned to the local formulary.
Ensuring the achievementof targets and budgets.
Liaising with Pharmacistsworking within the Leicester, Leicestershire and Rutland ICB
As a self-motivated andenthusiastic Clinical Pharmacist, the successful candidate will enjoy:
Work as part of a multi-disciplinary team in a patient-facingrole.
Help support the repeat prescription system, deal with acute prescriptionrequests, and medicines reconciliation on transfer of care and systems forsafer prescribing, providing expertise in clinical medicines advice while addressingboth public and social care needs of patient in the GP practice (s).
Take responsibility for areas of chronic disease managementwithin the Practice and undertake clinical medication reviews to proactivelymanage patients with complex polypharmacy.
Provide primary support to general practice staff with regardsto prescription and medication queries.
Provide clinical leadership on medicines optimisation, qualityimprovement, maintaining expenditure within set budgetary limits and managesome aspects of the quality and outcomes framework and enhanced services.
Ensure that the practice integrates with community andhospital pharmacists to help utilise skill mix, improve patient outcomes,ensure better access to healthcare and help manage workload.
Therole is pivotal to improving the quality of care and operational efficienciesso requires motivation and passion to deliver excellent service within generalpractice.

TelephoneAcute Medicines Support
Provide a telephone help line for patientswith questions, queries and concerns about their medicines and acutemedication requests.
To reconcilemedicines following discharge from hospitals, intermediate care and into carehomes, including identifying and rectifying unexplained changes and workingwith patients and community pharmacists to ensure patients receive themedicines they need post discharge.
Set up andmanage systems to ensure continuity of medicines supply to highrisk groups ofpatients (e.g. those with medicine compliance aids or those in care homes).

PatientFacing Long-Term Conditions Clinics
See patients face to face, or work withpatients on the telephone, or by written correspondence (where appropriatewith single or multiple medical problems where medicine optimisation isrequired (e.g. COPD, asthma).
Review the on-going need for each medicine, areview of monitoring needs and an opportunity to support patients with theirmedicines taking ensuring they get the best use of their medicines (i.e.medicines optimisation). Make appropriate recommendations to GPs for medicineimprovement.
PatientFacing Clinical Medication Review
Undertake clinical medication reviews withpatients and produce recommendations for Nurses and/or GP on prescribing andmonitoring.
PatientFacing Care Home Medication Reviews
Undertake clinical medication reviews withpatients and produce recommendations for the Nurses or GPs on prescribing andmonitoring.
Work with care home staff to improve safetyof medicines ordering and administration.
PatientFacing Domiciliary Clinical Medication Review
Undertake clinical medication reviews withpatients and produce recommendations for the, Nurses and GPs on prescribingand monitoring.
Attend and refer patients tomultidisciplinary case conferences.
Provide patient facing clinics for those withquestions, queries and concerns about their medicines in the practice
MedicineInformation To Practice Staff And Patients
Answers relevant medicine -related enquiries from GPs, other practice staff, otherhealthcare teams (e.g. community pharmacy) and patients with queries aboutmedicines.
Suggesting and recommending solutions.
Providing follow up for patients to monitorthe effect of any changes.
UnplannedHospital Admissions
Review the useof medicines most commonly associated with unplanned hospital admissions andreadmissions through audit and individual patient reviews.
Put in placechanges to reduce the prescribing of these medicines to highrisk patient groups.
Signposting
Ensure thatpatients are referred to the appropriate healthcare professional for theappropriate level of care within an appropriate period of time e.g. pathologyresults, common/minor ailments, acute conditions, long term condition reviewsetc.
RepeatPrescribing
Manage therepeat prescribing reauthorisation process by reviewing patient requests forrepeat prescriptions and reviewing medicines reaching review dates andflagging up those needing a review.
Ensure patientshave appropriate monitoring tests in place when required.
RiskStratification
Identificationof cohorts of patients at high risk of harm from medicines throughpre-prepared practice computer searches.
This mightinclude risks that are patient related, medicine related, or both.
ServiceDevelopment
Contributepharmaceutical advice for the development and implementation of new servicesthat have medicinal components (e.g. advice on treatment pathways and patientinformation leaflets).
InformationManagement
Analyse,interpret and present medicines data to highlight issues and risks to supportdecision making.
MedicinesQuality Improvement
Undertakeclinical audits of prescribing in areas directed by the GPs, feedback theresults and implement changes in conjunction with the practice team.
Implementchanges to medicines that result from MHRA alerts, product withdrawal andother local and national guidance.
ImplementationOf Local And National Guidelines And Formulary Recommendations
Monitorpractice prescribing against the local health economys RAG list and makerecommendations to GPs for medicines that should be prescribed by hospitaldoctors (red drugs) or subject to shared care (amber drugs).
Assistpractices in seeing and maintaining a practice formulary that is hosted onthe practices computer system.
Auditingpractices compliance against NICE technology assessment guidance.
Providenewsletters or bulletins on important prescribing messages.
EducationAnd Training
Provideeducation and training to primary healthcare team on therapeutics andmedicines optimisation.

Work with thegeneral practice team to ensure the practice is compliant with CQC standards.
