Job summary
A major healthcare provider in London is seeking a Network Navigator for its Acute Mental Health Wards. This full-time, in-person role involves coordinating patient care, ensuring timely discharges, and liaising with multidisciplinary teams. Applicants should hold a diploma in mental health practice or equivalent qualifications, along with significant experience in managing patients with serious mental illness.
