The Specialist Community Forensic Team (SCFT) have an exciting opportunity for a highly motivated Senior Mental Health Practitioner.

We a looking for a practitioner with a clinical speciality in a psychosocial intervention to work as part of the SCFT MDT supporting individuals in secure care to transition to community settings in West Yorkshire.

The service is now five years old, and the community caseload is growing. There are real time opportunities to deliver highly complex care and treatment in a range of settings to individuals who have significant health inequalities.

We use a trauma informed formulation led approach to our care and treatment, and work collectively as an MDT in decision making and risk management. The post comes with opportunity for autonomous practice, a package of training to support the role, and an embedded supervision structure. Additionally, the post holder will be part of the adult forensic community teams SLT, and as such this post will include development of leadership skills for the successful candidate.

Forensic Community Services across West Yorkshire are developing to reduce the length of stay in secure care, and to offer novel and previously unavailable care and treatment to individuals with high levels of need. This post will be part of supporting the vision for West Yorkshire as a whole at an exiting time of service development.

Main duties of the job

The All-Age Forensic Community Service is a developing service line within the Forensic Care Group. It also includes:

Forensic CAMHS (inclusive of Y&H FCAMHS, Focus Secure and Focus Wakefield)

Forensic Outreach and Liaison Service for Learning Disability and/or Autism (FOLS LDA)

Our adult forensic community service (Forensic Community Transition Team (Bretton Centre) and the Specialist Community Forensic Team (SCFT)).

The vision is to create a collaborative environment between our community forensic services and to develop and overarching clinical framework that will support the way in which a community forensic population is understood, supported and intervened upon, no matter what age. The purpose of the role is therefore to:

Be the lead professional in the case management of service users, providing advice and support to members of the MDT.

Undertake comprehensive assessments & risk assessments

Be enthusiastic about this service user group and motivated to support change and transition needs of individuals.

Able to demonstrate the ability to work autonomously and collaboratively with other agencies to make clinical judgements promoting joint decision making about care delivery to individuals with complex presentations.
