Job summary
A local authority agency is seeking a candidate for a 2-month contract role in Walthamstow to coordinate and broker care services for Adult Social Care residents. The ideal candidate will have experience working within Adult Social Care, knowledge of care markets, and capability to liaise with multidisciplinary teams. This role requires strong coordination skills to ensure timely hospital discharges and effective care placements. The position offers flexible working hours with one day in the office and bi-weekly payments.
