The role involves providing high-quality nursing assessments and support to residents in care homes to prevent hospital conveyance. Collaboration with community health teams and effective care planning are essential. To work as part of the Community Integrated Locality team and alongside staff in Identified Care Homes to provide a high quality, clinically robust, personalised assessment of all residents identified as high risk of conveyance to Hospital. Working in partnership with key staff members in both the homes and community teams to ensure each identified resident has implemented a clear care plan to manage their health needs, to include a clear pathway of how to contact Sutton Community Health services (SCHS) to prevent avoidable conveyance to hospital,
