An exciting opportunity has arisen for a highly motivated and experienced nurse to join our Critical Care Outreach Team. This Band 6 Critical Care Outreach Practitioner role offers an excellent chance to deepen your clinical expertise while developing advanced examination and diagnostic skills.
Our Critical Care Outreach Team delivers a proactive, 24-hour service focused on the early recognition, assessment, and management of deteriorating patients across the Trust. We support every stage of the acutely and critically ill patient pathway, from facilitating timely admissions to intensive care when required, to providing effective post ICU follow-up, and offering education that strengthens the skills and confidence of multidisciplinary ward teams caring for at-risk patients.
We are seeking an enthusiastic, dedicated practitioner with strong assessment, communication, and leadership abilities, who is committed to delivering outstanding, high-quality care outside the critical care environment. Improving patient safety is central to this role, alongside the opportunity to further develop advanced clinical skills in alignment with national standards and competency frameworks.
The post holder will become a key member of the Critical Care Outreach Team, working under the supervision of a Senior Outreach Practitioner.
They will develop advanced clinical assessment and diagnostic skills to identify, manage, and initiate early interventions for acutely or critically ill and deteriorating patients. Working within their defined scope of competence, the post holder will progressively acquire and apply specialist clinical skills under appropriate supervision.
The post holder will actively engage in a structured training and development programme to achieve advanced clinical competencies, enabling them to formulate and implement care plans for acutely and critically ill patients. They will also support the development of clinical skills and promote learning opportunities for staff across all professional grades within the Trust.
As required, the post holder will participate in the management of Level 2 and 3 patients outside of Critical Care Units, working closely with the multidisciplinary Critical Care team. They will coordinate the safe transfer of patients to appropriate care areas—both within the organisation and externally—once suitable beds are available. The post holder may also be required to work within Critical Care Units across all Trust sites.
