Description: Full-time PRIVATE Dentist required for 4 days per week FROM DECEMBER in a busy, mixed, well-established PRIVATE Independently owned practice near Evesham, Worcestershire.

1. £9K-£12K PER MONTH
2. 45% Remuneration
3. ESTABLISHED LIST TO TAKE OVER
4. Labs: 50% split
5. Tues 9am-6pm & Fri 9am-2pm
6. 7 Fully computerised surgeries with R4 software
7. Treatment Coordinator
8. Clinical Freedom
9. Hygienist
10. Digital X-rays
11. Intra oral cameras
12. Rotary endo
13. OPG
14. Performer number would be an advantage for a small kid's contract but not essential
15. 40 minutes from Cheltenham

3. GDC certificate
4. UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate

7. Enhanced DBS (within the last 12-36 months)
8. Clinical references covering places of employment over past 2 years (minimum 2 referees)
9. Degree certificate
10. CPR/AED/Medical Emergencies certificate dated within the last 12 months
11. IRMER
