A healthcare provider in Stockport is looking for a motivated clinician to join their Discharge Teams. You'll facilitate patient transfers from hospitals and community beds, ensuring smooth transitions and timely discharges. The ideal candidate will have strong communication and negotiation skills, along with the ability to work collaboratively within multidisciplinary teams. Join us to contribute to new initiatives that improve patient outcomes and help develop staff.
The post holder will work as part of the Discharge Teams to facilitate transfers from hospital and community beds, working in partnership with colleagues across health and social care to reduce length of stay and improve outcomes for patients and carers. We are looking for a highly motivated and enthusiastic clinician who will provide professional and clinical support within the Discharge Teams and community beds as the service requires, which can include the Transfer of Care Hub, the Transfer of Care Team, Discharge to Assess, and the Intermediate Care at Home Team. This is a really exciting time to join our team to become part of our integrated service as we have many new and exciting initiatives to challenge and develop our staff.
The candidate should demonstrate strong communication/negotiating skills and be able to work closely with members of the MDT and other stakeholders to facilitate timely and safe discharges to aid patient flow within the hospital and the community beds.

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary. In addition, it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16.
