Community Learning Disability Nurse

An exciting opportunity has arisen for a Community Learning Disability Nurse to work within the West CIDT, covering Mansfield and Ashfield, Nottinghamshire. The main base will be Portland Street, Kirkby-in-Ashfield.

The Intellectual Disabilities Service, Specialist Services Directorate provides specialist NHS services to people for whom mainstream services cannot meet their needs. We are seeking a Nurse to join our well established Community Nursing Team.

Main duties of the job

The successful candidate must be an RNLD with a Diploma or Degree level in Learning Disability Nursing and be registered with the NMC.

You will be highly motivated team player able to demonstrate a high level of professional competence. You will be expected to also work autonomously and be responsible for your own caseload. Excellent interpersonal and communication skills are essential. The post requires close work with patients, carers, external agencies and other health and social care professionals. As part of a team you will be expected to contribute to care plans alongside the risk management plans.

You must have the ability to analyse and assist in making decisions regarding client risk and support in complex cases.

You will be required to manage a busy caseload and carry out relevant physical and mental health assessments.

You will participate fully as a multidisciplinary team member working in partnership with social care and voluntary/independent sector providers. You will provide holistic advice and health education to service users, their carers and community support agencies. The post holder must be able to demonstrate awareness of current issues and legislation affecting people with Intellectual Disabilities.

Job responsibilities

Applicants must be an RNLD/ have a Diploma in Learning Disability Nursing and be registered with the NMC.

You will be responsible for managing a busy caseload.

To work autonomously and also as a member of the team.

To work to provide assessment and treatment to individuals within the community who may be experiencing a decline in their physical health, mental health and wellbeing, or displaying behaviours that challenge.

To work with patients, families, carers, social care and external providers.

You will participate fully as a multidisciplinary team member working in partnership with social care and voluntary/independent sector providers.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
