A regional healthcare provider is seeking a Deputy Community Nurse Team Lead for the Toressa Community Nursing Team in Saltash. An exciting opportunity to join our organisation as a Deputy Community Nurse Team Lead for the Toressa Community Nursing Team in Torpoint and Saltash.
We're an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people's physical and mental health. We also provide specialist support to people with dementia or a learning disability.

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation.
The post holder will work in this adaptable and evolving role, focusing on safe, effective admission avoidance and supporting timely discharge from hospitals and other care settings. Implementing personalised support plans and collaborating with the whole community resource to facilitate ongoing care management.
To work in partnership with patients and carers and members of the multi‑disciplinary team to offer support and advice and promote informed choice; maximising independence and optimising quality of life. This post holder will be required to provide clinical management and leadership to all staff within the team.
The post holder will either possess the Long Term Conditions qualification or be willing to complete it when in post.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
