Band 5 Community Staff Nurse

We are looking to recruit a dynamic and innovative Band 5 Community Staff Nurse to work within the Beverley, West Wolds and Hornsea Community Nursing Team for the Integrated Nursing and Conditions services within CHCP CIC.

Overview

The successful applicant will join an integrated community nursing team delivering holistic high‑quality nursing care to a dedicated client group. You will be part of a dynamic and proactive nursing team working in partnership with associated health and social care providers, with opportunities for developing new ways of working and working under your own initiative. Experience of community nursing is advantageous but not essential as full training, support and development will be provided.

Responsibilities
  • Deliver holistic, high‑quality nursing care to a dedicated client group.
  • Work within an integrated community nursing team, collaborating with associated health and social care providers.
  • Develop and implement nursing plans in partnership with patients, families and multidisciplinary teams.
  • Provide education and support to patients and carers to promote self‑management and wellbeing.
  • Maintain accurate and timely documentation in clinical records.
Qualifications & Requirements
  • Band 5 nursing qualification and registration with the Nursing and Midwifery Council.
  • Experience of community nursing is advantageous but not essential.
  • Full training, support and development will be provided.
  • Ability to commute within the geographical area with access to appropriate transport.
  • Strong communication and teamwork skills.
