Overview

Adult Social Care Review and Transformation Team - Full time / 6 months fixed-term contract

The Review and Transformation team are looking for an enthusiastic and creative social worker to join our team. The team covers the whole borough of Sefton, and focuses on assessment and review activity, and a number of specialist projects, as part of the transformation programme - with a focus on promoting independence, prevention and sustainability. This is an ideal opportunity for a newly qualified social worker. The successful applicant must have a Social Work degree or equivalent qualification, and be able to register/be registered with Social Work England.

Responsibilities
  • Experience of strength-based Care Act assessments and outcome-focused support planning, assessing mental capacity and making applications for health funding is essential.
  • The successful applicant must have a passion for promoting person-centred care and a commitment to professional development.
  • Applicants with backgrounds in community work, and experience of working with service users with learning disabilities and dementia would be at a distinct advantage.
  • It is essential that you are able to manage and prioritise your own caseload, think creatively, and work flexibly and from your own initiative. You will be able to see cases through to completion and resolution.
  • In this role you will have excellent interpersonal skills in order to effectively undertake partnership working with all of our stakeholders, and further develop these relationships.
  • You will be part of a creative, and welcoming team. You will have a comprehensive induction programme, be provided with structured support and training opportunities, receive monthly supervision, and have a protected caseload.
Requirements
  • You will be exempt from Rehabilitation of Offenders Act 1974 due to the nature of work; appointment subject to satisfactory Enhanced Disclosure from the Criminal Records Bureau.
