We are seeking a dedicated and experienced Social Worker to join our dynamic Adults Locality Team in Oldbury. This ongoing locum role is ideal for professionals passionate about improving outcomes for adults with varying needs including older adults, individuals with physical or sensory impairments, learning disabilities, and mental health conditions. Manage a varied caseload involving adults with complex and diverse needs.
Undertake Care Act 2014 assessments and reviews to deliver personalised support plans.
Conduct triage calls, manage referrals, and deliver timely interventions to support community independence.
Complete a minimum of three case-related tasks per week to maintain service efficiency.
Attend team meetings every other Friday to support shared learning and service alignment.
Contribute to waiting list reduction and case review initiatives to enhance access to support.
Degree in Social Work (DipSW, CQSW, or equivalent)
Social Work England registration
Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, Community DoLS/LPS, Safeguarding, and Continuing Health Care
Experience working in a fast-paced, high-volume adult services setting
Confident communicator with excellent organisational and documentation skills
