Job description
Overview

This job is with the Homeless Intermediate Care Team (HICT) at University College London Hospitals NHS Foundation Trust (UCLH). The team supports people experiencing homelessness with health and social care support and temporary accommodation after discharge from hospital. The vision is to end the cycle of hospital discharge to rough sleeping and rapid re-presentation to emergency departments.

The HICT aligns to Discharge to Assess (D2A) principles, focusing on patients discharged from UCLH, Royal Free and Whittington hospitals and their D2A P2 stepdown beds.

Job responsibilities

The team provides case management for assessment, treatment and assertive signposting to existing services outside the hospital setting for a time-limited period. It works with adults who are homeless, rough sleepers and those at risk of rough sleeping including those with no recourse to public funds (NRPF). The post-holder will be a core member of HICT, delivering and coordinating operational output.

The role supports the service to address acute and chronic needs of medically optimised patients through holistic health needs assessment and by facilitating access to long-term health care and/or housing. The post holder will work within a multidisciplinary-led service to ensure a timely, safe discharge to temporary housing or a health provider setting. The service is aligned to D2A principles, providing assessment and ongoing health and social care support linked into mainstream homeless services. Patients have varying levels of complex needs including substance misuse, alcohol and mental health problems.

The post holder will collaborate with the Clinical Operations lead and other senior managers to help reduce discharge delays from step-down accommodation and hospitals for the homeless population. The role includes gathering/recording information to contribute to an evidence base for a holistic multidisciplinary wrap-around service within discharge-to-assess pathways. It also involves monitoring processes, implementing changes where needed and maintaining high standards while representing UCLH.

Person Specification

Knowledge and Qualifications

Essential

  • Educated to degree level in a relevant subject or equivalent level of experience in a similar role
  • Registered Health Care Professional (e.g., NMC for nurses, GPhC for pharmacists, or other relevant registration)
  • Knowledge of issues that characterise health services for vulnerable groups and homeless persons
  • Evidence of continuous personal and professional development and willingness to undertake ongoing CPD

Desirable

  • Further training or experience in a clinical supervisory position
