Salary: £44,951 - £51,064 per annum plus £2,000 Golden Hello and a £2,000 Annual Retention Allowance. Location: Churchill Court (and hybrid working). Contract Type: Permanent. Hours: Full Time (36 hours).
We are looking for an experienced Senior Social Worker to join our friendly and hard-working team as part of the new ASC front door model. The role involves working in conjunction with the Initial Contact Team and Safeguarding Team, based in the Community Intervention & Prevention Hub. The front door offers focused short-term intervention to mitigate immediate risk and ensure that residents wait well when further complex care management has been identified. You will work alongside OTs in this new MDT approach, responding daily to referrals via the hub which deal with a broad range of Adult Social Care queries and referrals. There is focus on supporting the Integrated Care Network (ICN) by working collaboratively with health partners to offer short-term intervention in the community.
