Overview

We have an exciting opportunity for a Practice Nurse to join our Primary Care team at HMP Hatfield and The Lakes, both category D Prison and Young Offenders Institution located in South Yorkshire.

Hours

28 hours per week working Monday to Sunday, 9 hour shifts, 7.30am-5pm, split across two sites weekly. One in 3 weekends. No nights.

This position is part-time, you’ll receive an annual salary up to £40,211 per annum FTE - depending on experience.

Candidates are required to have a Legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.

Main duties of the job

As a Practice Nurse, your responsibilities will vary. You will:

  • Work together at each stage of the patient journey, supporting them as they arrive into prison (Early Days in Custody Team), delivering the care they need, promoting good health and supporting resettlement (Planned Care Team) and responding to urgent and emergency needs (Unscheduled Care Team).
  • Provide clinical leadership for the Primary care team and be a critical leader within the scheduled care Team. This multi-disciplinary team is made up of a variety of registered and non-registered professionals including; nurses, doctors and mental health/substance misuse practitioners.
  • Deliver face to face care and provide safe and effective health assessments and treatment of patients.
  • Be accountable for your own practice, including the assessment, diagnosis, treatment and discharge or referral of patients, independent of medical intervention.
  • Lead in complex wounds, long-term conditions and QoF management.

Qualifications
  • Registered Nurse: Adult Nursing with current NMC registration
  • Experience working autonomously as a clinical practitioner following best practice guidance and identifying risk to patients through sound use of clinical expertise
  • Knowledge of clinical audit and research
  • Knowledge of the statutory requirements for the safeguarding of adults and children
  • Willing to be innovative and embrace learning through continued professional development and the practical application of new skills in the workplace
  • You need expert clinical skills and demonstrate judgement in assessing and managing wide ranging and complex patient problems

Although not essential, ideal applicants would have experience in a hospital, community or prison environment.
