Overview

We are looking for an enthusiastic and reliable Practice Nurse to support our practice team in delivering primary care service to our diverse patient population. You will be working with a varied patient demographic and an experienced multi‑disciplinary team.

Main duties of the job

The ideal candidate will have experience in the core competencies of practice nursing including wound care, child immunisation, independent and proactive chronic disease management (in particular diabetes) and cervical cytology. In addition, experience of management responsibilities would be beneficial, as there will be scope for the successful applicant to take on additional individual organisational and management responsibilities.

Job responsibilities

Job summary:

Helping the practice develop and deliver specialist primary nurse led services including chronic disease management. Make professionally autonomous decisions and provide safe, evidence based, cost effective, individualised patient care.

Job responsibilities:

Providing assessment, screening and treatment services and health education advice

Cytology

Wound care

Travel Clinic

Asthma/Peak Flow Readings

Routine immunizations/Childhood immunisations

Requesting pathology tests, for example urine culture, swabs

Diabetic reviews

Other tasks will include:

  • Maintain general tidiness and cleanliness of nurses and treatment rooms
  • Maintain stock of waiting room leaflets as instructed by partners and Practice Manager
  • Ordering of vaccinations to maintain stock levels
  • Attend and participate in any Practice meetings when required
  • Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc. and ensure the cleanliness of any equipment used
  • Perform any investigatory procedures requested by the GPs
  • Ensure accurate notes of all consultations and treatments are recorded in the patient notes on the clinical computer system as appropriate
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
  • Attend and participate in practice meetings as required
  • Maintain a notice board in the waiting area designated for patient health care education
Person Specification
Qualifications
  • Qualified Nurse

Experience
  • Experience working within a nursing team with clinical and non-clinical colleagues providing an optimum level of care delivery to patients.
  • Experience of taking and maintaining responsibility for the nursing components of care.
