Overview

We are seeking an experienced and passionate Patient Safety Manager to join our dynamic Quality and Patient Safety Team at Herefordshire and Worcestershire Health and Care NHS Trust. This is an exciting opportunity to play a critical role in leading and developing our patient safety agenda across the organisation. As a key leader within the Trust, you will manage and coordinate the delivery of a safe, responsive, and effective patient safety service, ensuring compliance with national standards and statutory requirements. You will support the implementation of the Patient Safety Incident Response Framework (PSIRF) and provide expert guidance, leadership, and oversight of incident management processes, driving continuous improvement and learning across clinical and corporate services.

Responsibilities
  • Lead and manage the Patient Safety Team, including line management of the Patient Safety Coordinator and Administrator.
  • Act as the Trusts expert in patient safety, ensuring statutory compliance and high-quality service delivery.
  • Support and deputise for the Patient Safety Service Lead, and oversee daily incident triage and investigation processes.
  • Provide expert advice to senior managers and clinical teams in relation to patient safety events, learning, and improvement.
  • Analyse, evaluate, and report on patient safety data to identify trends, risks, and opportunities for change.
  • Promote a proactive and positive culture of safety and learning, including developing and delivering training.
  • Engage with internal and external stakeholders, including service users and system partners, to ensure excellence in patient safety outcomes.
  • Contribute to the wider quality governance agenda, aligning patient safety with complaints, claims, safeguarding, and regulatory compliance.
