A leading healthcare provider in mental health is seeking a Senior Mental Health Practitioner to work within the Living Well service in Tameside. An exciting opportunity for a senior mental health practitioner to work within the Living Well service in Tameside & Glossop. The role involves working collaboratively with individuals in a community setting to help them navigate and connect to a range of mental health services, community activities and services in line with their individual needs, circumstances and goals to enhance their health, wellbeing and quality of life.
• Offer initial conversation and biopsychosocial assessments.
• Collaboratively engage in person‑centred next‑step planning.
• Screen referrals coming through the open door and devise risk management plans with service users as part of the duty function.
• Promote self‑management to enable service users' engagement with other parts of LWT and offer advice to colleagues where necessary.
• Deliver brief interventions (max 12 weeks) including coping strategies, problem‑solving, motivational and behavioural activation work, anxiety management, risk management and suicide prevention work, medication support, promoting social and inter‑agency support, and providing psychological support and biopsychosocial education aimed at insight development.
• Provide self‑help resources and signposting, and review treatment plans with service users deemed too complex for other parts of LWT or who may not meet the criteria for secondary care services.
• Applicants must possess RMN / RNMH / Social Worker / OT or equivalent.
• Completion of Preparation for Mentorship / Practice Teacher is desirable together with significant post‑registration experience.
