A healthcare provider in England seeks a Health Care Assistant to deliver quality patient-centered support in the community. The role involves working with adults over 18, ensuring their needs are met through effective communication and care plans. You will be part of a MDT team in which you will receive clinical and managerial support. This role is part of a wider MDT aligned to social care professionals in reflection of the NHS 10 year plan.
You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff you will be working alongside. You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.
