Job description
Overview

You will be part of a MDT team in which you will receive clinical and managerial support. This role is part of a wider MDT aligned to social care professionals in reflection of the NHS 10 year plan.

You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff you will be working alongside. You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.

Responsibilities
